Documents & Handbooks.
Our registration for 2011-2012 has
begun! As a program we are trying our best to provide the best
teaching & training possible for a reasonable cost. Our Fees are as
follows:
2011-12 REO Registration

through September 9

50.00 / child (family maximum $200)
LATE FEE! $10.00 per
child. Students whose registrations
arrive in our office after September 9:

will incur the $10 per child late fee

will not be admitted into any session until paperwork has
been processed.

will be notified
when they may begin to attend PSR sessions.
For those children who are going
to be prepared, by our office, to receive a sacrament
(Reconciliation, Eucharist or Confirmation) there is an
additional $20.00 fee to help defray the costs of the additional
materials required.
All checks should be made out to St.
Mary's Church.

Registration Letter
Below is a general instruction for
completing the registration form:
Choose a Ministry
- Across the top of the form are a
group of checkboxes. Select the box that represents your
desired ministry. Based on the selection, complete the
following sections:
If you select "PSR", "Teen Faith
Program", "Confirmation", "SMS Student" or "Home Schooled" complete
the following Information sections:
General Information.
This portion of the Registration Form
contains basic data about the individual. Name, address, phone
& email address are the data elements required in this portion of
the form.
Family Information.
This section contains data about your
parents. This data is used to verify Parish Registry
information. We ask for "full" baptismal names in order to
verify these records.
Student Information.
This portion of the form requests birth &
school data. Student email & Student Cell Phone is intended
for those high school & confirmation students as another way to
contact the student.
Sacramental Information.
This section requests the specific data
about the students Baptism & First Communion, if applicable.
If you choose "Catechist/Leader", "Children's Liturgy" or
"Aide/Volunteer" complete the following information sections:
General Information.
This portion of the Registration Form
contains basic data about the individual. Name, address, phone
& email address are the data elements required in this portion of
the form.
Leader Information.
This portion of the form contains the dates
that specific Tax and Diocesan forms & requirements have been met.
When returning your
registration document(s), please be sure to include the following:
1. Registration Form.
This form should be completed (or
updated), signed and dated including the medical authorization.
This form is on the reverse side of the Registration or page 2 of
above form.
2. Copy of Baptism
Certificate. This certificate
is required for ALL Confirmation Candidates and new PSR/Teen
Faith Students.

In addition to the above
forms/fees, Confirmation Candidates are required to provide the
following:
4. Sponsor Selection
Form. Each Candidate must
identify at least one person who agrees to be your guide, your
support, your catholic friend, throughout your Catholic life.
The form requests a name, address, phone, email and Parish.
This document will be provided at the Parent Meeting.
5. Sponsor Paragraph.
Each Candidate is required to
explain why he/she has chosen the person they did.
6. Confirmation Name
and Explanation. Each
Candidate is required to choose a name and write a paragraph
explaining their choice. This document will be provided at the
Parent Meeting.
8. Sponsor
Certificate. Each Sponsor is
required to provide proof that you are a member of a parish and
practicing Catholic. This certificate can be obtained by
contacting your church's parish office.
Return all completed forms/fees to
the Religious Education Office. All checks should be made out
to St. Mary's Church.
(*) It is the policy of St. Mary’s Church
that no child/candidate is refused instruction on the basis of need,
but texts and materials are expensive. Therefore, if fees cannot be
paid, parents or guardians must contact Father George Vrabel or the
Director of Religious Education prior to the registration deadline.
